Skin Treatments with Dr. Spiegl

Modern Aesthetic Dermatology in Salzburg and the Tullnerfeld

The methods in this category do not work on volume or tissue tension but on the skin itself: on its structure, its quality, its pigmentation and its capacity to regenerate. The result is therefore not a single outcome but, in most cases, the product of several sessions and of the home care that accompanies them – and in every case the product of consistent sun protection, which determines how long the result lasts more than the treatment itself does. Each course of treatment therefore begins with a detailed skin analysis and consultation, from which a tailored plan emerges, frequently as a combination of several methods. Dr. Matthias Spiegl is a board-certified specialist in Plastic, Reconstructive and Aesthetic Surgery and performs these treatments in Salzburg and the Tullnerfeld; the medical setting permits penetration depths and active-ingredient concentrations that are not permissible in the cosmetic sector. The overview below introduces the available treatments briefly – each detail page covers the procedure, downtime and costs.

Chemical Peel

In a chemical peel the uppermost layers of the skin are removed in a controlled manner: the active substances eliminate dead cells, stimulate cell renewal and promote collagen formation. Glycolic acid, lactic acid and trichloroacetic acid are used, applied superficially, at medium depth or deeply according to skin type and objective; the treatment addresses fine lines, pigment spots, sun damage, acne, enlarged pores, scars and an uneven complexion. A session takes 30 to 40 minutes and usually requires no anaesthesia; a numbing cream can be applied beforehand on request. Several sessions at intervals of two to four weeks are recommended; downtime depends on the depth and ranges from one to two days of redness with a superficial peel to approximately one week with crusting after a deep one. The final result becomes apparent after four to twelve weeks and lasts, depending on skincare and sun protection, six to sixteen months.

Chemical Depigmentation

Chemical depigmentation treats pigmentation disorders in a targeted way: melasma and chloasma, sun and age spots, and post-inflammatory hyperpigmentation, for instance following acne. Individually composed combinations of active ingredients – among them tranexamic acid, enzyme complexes, tyrosinase inhibitors and niacinamide – act on several fronts at once: they inhibit melanin formation, reduce inflammatory processes and promote skin renewal. The in-clinic session takes approximately 30 to 60 minutes; decisive for the result, however, is the home therapy that follows over three to six months, accompanied by consistent sun protection. The final result becomes apparent after six to twelve weeks and typically lasts six to sixteen months, although recurrence of the pigmentation remains possible. Lightening of the intimate area is not offered.

Vampire Lifting (PRP)

In vampire lifting a small quantity of the patient’s own blood is drawn and centrifuged to obtain platelet-rich plasma (PRP). The plasma is then injected into the skin, where the growth factors it contains stimulate cell regeneration and collagen formation; because autologous material is used, the risk of allergic reactions is low. The treatment addresses tired, dull skin, fine lines, declining elasticity and dark circles, on the face as well as the neck, décolleté and hands; a further application is the stimulation of hair follicles in cases of hair loss. It is performed without anaesthesia, optionally with a numbing cream, and at least three sessions are foreseen for a good result. The method is not suitable in cases of active skin infection, coagulation disorders or severe chronic illness, nor during pregnancy and breastfeeding; the minimum age is 18.

Microneedling

In microneedling, very fine needles create numerous microchannels in the skin. The repair response this triggers increases the body’s own production of collagen and elastin, and the active ingredients applied penetrate far more deeply through those channels. The work is done with a pen that punctures the skin several times per second with between 8 and 36 needles; the depth is set case by case and usually lies between 0.7 and 1.5 millimetres – deeper, that is, than is permissible in the cosmetic sector. The treatment addresses enlarged pores, atrophic and hypertrophic scars following acne, surgery or injury, a dull complexion, dry skin and fine lines. After cleansing and the application of a numbing cream the session takes a few minutes; around three sessions at intervals of several weeks are usually recommended, and combination with PRP is possible. Redness and mild swelling generally resolve within days, a period in which sun protection is decisive.

CO₂ Laser Treatment

Fractional CO₂ laser treatment removes the finest layers of skin selectively while simultaneously stimulating new collagen formation at depth: it is the most intensive of the methods described on this page and, accordingly, the one with the greatest effect. It addresses fine lines and deeper wrinkles, photoageing and sun damage, scars following surgery or acne, pigmentation disorders and age spots, enlarged pores and a dull complexion. A numbing cream is applied before treatment; sun and sunbeds are to be avoided for at least four weeks beforehand, blood-thinning medication and irritant products are to be discussed in advance, and any existing inflammation must have resolved. The skin peels for approximately five days and is healed after about a week; slight residual redness may persist for some weeks and can be covered with make-up. A single session can already produce marked effects, while a series is appropriate for more pronounced changes.
