Facial Surgery with Dr. Spiegl

Aesthetic Facial Procedures in Salzburg and the Tullnerfeld

The face is where ageing shows most visibly – and at the same time the area in which even small changes shape the overall expression. Declining skin elasticity, descending tissue and loss of volume make the eyes look tired and the contours indistinct, often years before a person feels old. Aesthetic facial surgery therefore does not aim at a transformation, but at restoring one’s own original form. Dr. Matthias Spiegl is a board-certified specialist in Plastic, Reconstructive and Aesthetic Surgery and performs these procedures in Salzburg and the Tullnerfeld – a large proportion of them on an outpatient basis under local anaesthesia. Which procedure is appropriate is decided by the findings: an eyelid lift will not open up the eyes if the cause lies in descended brows, and fillers are no substitute for a lift once the deeper tissue has given way. The overview below introduces the available procedures briefly – each detail page covers technique, surgical course, aftercare and costs.

Upper Eyelid Lift (Blepharoplasty)

The upper eyelid lift removes excess skin from the upper lid and, where required, small fat deposits. It is indicated in cases of hooded eyelids – whether congenital or age-related – a tired or sad expression despite sufficient sleep, and a restriction of the visual field caused by the skin excess. The portion to be removed is marked before surgery to secure a result that is as symmetrical as possible; the scar lies in the natural lid crease and is barely visible once healed. The procedure takes 45 to 60 minutes under local anaesthesia, the sutures are removed after approximately seven days, and contact lenses should be avoided for at least two weeks.

Lower Eyelid Lift

The lower eyelid lift corrects tear troughs, excess skin and the tired-looking shadows beneath the eyes; it is frequently combined with an upper eyelid lift. Depending on the findings, the lift is performed either through an external access or, in suitable cases, as an internal lower eyelid lift with no visible external scar. The procedure is usually carried out under local anaesthesia, alternatively under twilight sedation; for more extensive findings, general anaesthesia is recommended. Approximately half an hour per eye should be expected, sutures and dressing strips are removed after about a week, and sport and physical exertion should be avoided for at least two weeks.

Eyebrow Lift

The eyebrow lift raises descended brows and opens up the eyes – often precisely in cases where an upper eyelid lift alone is not sufficient, because the cause lies higher up. Three techniques are available: the direct lift, removing a fine strip of skin above the brow; the endoscopic lift, through small access points at the hairline; and the gliding brow lift, in which the forehead tissue is mobilised and repositioned by sliding. Dr. Spiegl favours the gliding brow lift, in which the temporary fixation sutures remain in place for approximately three to four days. The procedure takes one to two hours, is performed on an outpatient basis under local anaesthesia, twilight sedation or general anaesthesia, and requires no drains.

Lip Lift (Bullhorn Lift)

The lip lift shortens the distance between nose and upper lip, which lengthens over the years. A narrow strip of skin is removed directly beneath the nose: the upper lip is slightly raised and more of the vermilion remains visible – without injectables. The scar lies in the natural crease beneath the nose and is barely visible once healed. The procedure takes approximately one hour under local anaesthesia and is performed on an outpatient basis; patients are fit for work after around four days, and the result becomes apparent after two to three months.

Facelift & Necklift

In a facelift it is not the skin that is pulled tight, but above all the deeper muscle and connective tissue layer (SMAS) that is repositioned – which is why modern techniques place less tension on the skin and look more natural. The procedure addresses jowls, a jawline that has lost its definition and a lax neck; the neck lift alone is also possible as an isolated procedure, and lost volume can be replaced in a targeted manner with autologous fat. The incisions run within the hairline and around the ear. Surgery takes two to five hours under general anaesthesia depending on its extent; a compression mask is usually worn in the early healing phase, patients are fit for work after two to three weeks, and the final result becomes apparent after three to six months.

Ear Pinning (Otoplasty)

Otoplasty reshapes the auricles so that they lie closer to the head and appear more symmetrical; depending on the findings, the cartilage is modelled, reduced or fixed in position, occasionally supplemented by folding of the auricle. The incisions lie behind the ear and the sutures are removed after approximately twelve days. The procedure is performed under brief general anaesthesia or, where the findings are mild, under local anaesthesia; it takes approximately one to one and a half hours and is carried out on an outpatient basis. In children it is possible from around five to six years of age, once ear growth is largely complete; a soft headband is then worn for two to three weeks.
